HSE apologises over delay in acting on X-ray showing man aged 91 had swallowed dentures

The HSE has apologised to the family of a 91-year-old man who died with choking complications after a hospital took a week to act on an X-ray that revealed he had swallowed dentures. At the High Court sitting in Cork, Ms Justice Carmel Stewart approved a €55,000 award on Wednesday to the family of Daniel O'Leary, late of Knockbrack in Banteer, Co Cork. The court heard how on July 26th, 2022, Mr O'Leary, a resident at Kanturk Community Hospital, Co Cork, started coughing while sitting in a chair. He was uncomfortable and complained about something stuck in his throat. READ MORE He was holding his hands to his throat and his partial bottom denture of two teeth was missing. Staff went to his aid and he was transferred to University Hospital Limerick. Paramedics told doctors he had possibly inhaled a denture consisting of two lower teeth. He was examined in the emergency department, and X-rays showed no evidence of the dentures or any foreign body. Mr O'Leary was admitted to hospital. He was further assessed the following day and again a doctor found no evidence that his upper airway was obstructed. A diagnosis of respiratory sepsis was made on a background of likely aspiration due to impaired swallow. Mr O'Leary was given fluids and antibiotics intravenously. He also received oxygen therapy. He was assessed two days later when he appeared to be improving. He remained in hospital over the bank holiday weekend, with his care being taken over by a consultant physician and geriatrician on August 2nd. Ms Justice Stewart was told a chest X-ray carried out on his arrival at the hospital on July 26th was only reported on July 30th. It showed 'densities in the upper neck, likely reflecting a foreign body'. The court was told there was no evidence the X-ray report had been seen or acted on by medics in the hospital until the consultant geriatrician came on duty on August 2nd. A CT scan carried out on August 3rd did not detect any foreign body in Mr O'Leary's neck or chest. However, later that day nursing staff carried out a suctioning procedure on the patient and detected dentures in the lowest part of his throat. The ENT team removed the dentures on August 4th, 2022, nine days after he was taken to hospital. Mr O'Leary continued to deteriorate physically and his care became palliative. He was brought back to Kanturk Community Hospital where he died peacefully on August 12th, 2022. Eamon Shanahan, for his family, said they received an apology from the HSE. In the apology the health service acknowledged the delay in the review of the relevant X-ray and said it has 'learned from the incident' and implemented a recommendation identified by the coroner at the inquest concerning Mr O'Leary's death. 'We wish to apologise for the failings in the care provided and for the distress and upset that this has caused,' it said. In effect there was a week between the X-ray, which showed a potential blockage, and when it was acted upon. Ms Justice Stewart extended her condolences to Mr O'Leary's family. 'Your father had a long and fruitful life,' she said. 'There is never a good time to lose a loved one, or to endure the trauma he endured. It doesn't bear thinking about. Hopefully you can put this behind you and reflect on the good times.'

Eddie Dunbar withdraws from Tour de France with wrist injury

Eddie Dunbar's maiden Tour de France has been cut short due to an injury suffered in a crash that occurred late in Friday's stage. 28-year-old Dunbar got caught up in a large pileup, and although the Banteer cyclist did continue thereafter, he has since been forced to opt out of the race, having not started Saturday's stage eight. Jayco Alula have said that Cork native Dunbar will withdraw from the Tour due to wrist pain. "Following a crash in the final kilometres of yesterday's stage, Dunbar has been suffering with pain in his wrist and a decision was taken to stop, for his safety." Dunbar had taken an impressive fourth-place finish in Thursday's sixth stage, which was won by Ben Healy.

Banteer Eddie Dunbar starts in Tour De France At noon last Sat, one of the most grueling sporting events in the world began, featuring 184 participants. The Tour de France cycling race has been running since 1903, but when this year's event began in Lille a little piece of history for Cork was made. For the first time, the ranks of cyclists will include a Corkman, in the shape of Eddie Dunbar, from Banteer. Eddie will be proudly donning the jersey of Team Jayco AlUla - the only Irish member in an eight-man line-up. Eddies main role is to provide back-up to his team leader, Australian Ben O'Connor, but the experience he gains may set him up for a good tilt at the event in the years to come. This is a great achievement for Cork, in a race where Ireland has had some stand-out moments down the years. ADVERTISEMENT Eddies achievement now inks the name of Cork further into the history of this famous event. This year's event lasts for 22 days and the cyclists will cover a total distance of 3,338km in 21 stages. The longest stage of this year's Tour de France covers more than 209km in one day, which is like cycling from Patrick Street to Gorey - very, very fast, at average speeds of around 40km/h. These super-fit cyclists will push themselves to the brink of exhaustion on the Tour. We wish Eddie the best of luck in the 3 week tour and you can follow each live on TV. It was a day full of fun, chat, and laughter -- a perfect reflection of Rory himself. A huge thank you to everyone who came to support the first Rory O'Connor Memorial Football Tournament. Well done to all the teams who took part, especially Croke Rovers, who defeated Dromtarriffe in the Cup final, and Sliabh Luachra Gaels, who overcame BK Plunketts in the Shield final. A special mention to our own fantastic U12 footballers, who were narrowly defeated by Dromtarriffe after extra time in a thrilling Cup semi-final. Heartfelt thanks to Pat, Bernie, Grace, Carole, and the O'Connor family for their incredible support in making the tournament possible. We look forward to continuing to celebrate Rory's memory again in 2027. Underage Soccer camp Banteer's Underage Soccer camp takes place on 23rd, 24th and 25th July. This is a coaching camp for boys & girls from 6 to 16 years approx. in their own groups. Time: 11am to 3pm. Registration starts at 10.30am on first day. Venue is Banteer Sports field. Fee: €35 per person paid on the first day. Camp is confined to Banteer Underage Soccer, the greater Banteer Community area and to those who always support the camp. Body found in Co Cork as gardaí appeal for information

Gardaí have appealed for witnesses after a body was discovered at Ballymaquirk, Mallow, Co Cork on Saturday evening. The witness appeal is seeking information in relation to a male walking on the road between Banteer and Kanturk, Co Cork, on the evening of Friday, June 13th between the hours of 3.30pm and 9.00pm. Road users who were travelling in the vicinity at the time who may have camera footage including dash cam are asked to make this available as investigations are ongoing. Anyone with information is asked to contact Kanturk Garda station on (029) 20680 or Mallow Garda Station on (022) 31450, the Garda Confidential Line on 1800 666 111, or any Garda Station.

Eddie Dunbar to ride his first Tour de France after being selected by Jayco Alula

The 28-year-old Corkman will be part of a dual-focused team built around Australian climber Ben O'Connor and Dutch sprinter Dylan Groenewegen. Dunbar, however, has shown he can race with the best in the peloton by winning two stages of the Vuelta a Espana last year and finishing seventh at the 2023 Giro d'Italia. Despite stepping up to the World Tour level almost eight years ago, the Banteer racer has had to wait for his chance to compete in the biggest race in the calendar. Dunbar finished the traditional pre-Tour warm-up race, the Criterium du Dauphine in 19th place overall after a mixed week but he had his early season interrupted after he suffered a concussion and a whiplash injury in a crash at Tirreno-Adriatico. O'Connor finished second in the Vuelta general classification last year and in 2021 was fourth at the Tour. The Australian team are eager to row in behind the Aussie rider, who finished seventh at the Tour de Suisse earlier this month. 'We're really looking forward to getting on the road at the Tour de France with Dylan Groenewegen for the sprints and we've got Ben O'Connor for GC. We'll have the national time trial champion and road race champion, Luke Plapp and Luke Durbridge, as Australians in the team, along with Ben, who is the first Australian GC leader for the team,' said the team's sports director Mathew Hayman.
