Ayushman Vay Vandana Scheme settles over 1.06 lakh claims: Govt

New Delhi: More than 1.06 lakh claims have been settled under the Ayushman Vay Vandana Scheme, said Prataprao Jadhav, Union Minister of State for Health and Family Welfare, on Tuesday. In October 2024, the scope of the Ayushman Bharat-Pradhan Mantri Jan Arogya Yojana (AB-PMJAY) was expanded to include all senior citizens aged 70 years and above, irrespective of their socio-economic status. Under this initiative, beneficiaries can receive free treatment benefits of up to Rs 5 lakh per year through Ayushman Vay Vandana cards. 'As on date, over 1.06 lakh claims have been settled under Ayushman Vay Vandana scheme since its launch,' Jadhav said. 'A total of 75.41 lakh Ayushman Vay Vandana cards have been created, out of which 32.3 lakh cards have been created for female beneficiaries,' he added. In addition, the portability feature of AB-PMJAY allows eligible beneficiaries -- including those under the Vay Vandana scheme -- to access healthcare services at any of the 31,466 empanelled hospitals across the country, regardless of their place of residence. 'This ensures seamless and equitable access to quality healthcare for the elderly population nationwide,' the MoS said. 'Vay Vandana scheme beneficiaries can also avail treatment through a vast network of 14,194 private healthcare providers empanelled under the scheme,' he noted. To ensure quality and consistency in service delivery, the National Health Authority (NHA) issued comprehensive Hospital Empanelment and Management (HEM) Guidelines for the empanelment of hospitals under the AB-PMJAY. The AB-PMJAY has emerged as one of the world's largest publicly funded health insurance schemes. It has enabled 8.59 crore hospital admissions worth Rs 1,19,858 crore, ensuring access to secondary and tertiary care without pushing families into debt, according to an official statement by the government. Meanwhile, Jadhav also shared the steps taken to strengthen the district-level health system, such as the Pradhan Mantri-Ayushman Bharat Health Infrastructure Mission (PM-ABHIM). 'PM-ABHIM has been launched with an outlay of Rs 64,180 crore over five years (FY 2021-26). The mission focuses on strengthening the capacities of health systems across all levels -- primary, secondary, and tertiary care -- to ensure effective response to current and future pandemics and disasters,' the Minister said. Under the Centrally Sponsored Scheme (CSS) components of PM-ABHIM, provision has been made to provide support for establishment of 602 Critical Care Hospital Blocks (CCBs) 50/100 bedded in all the districts with population of more than 5 lakhs during the scheme period i.e. from FY 2021-22 to FY 2025-26 including the ICU beds, he noted.

The State government's Medical Insurance Scheme for State Employees and Pensioners (Medisep), the first phase of which is drawing to a close on June 30, will be extended till September 30 with the same terms and conditions stipulated in the original Memorandum of Understanding with the insurance firm implementing the scheme. With the three-year policy period of Medisep coming to an end on June 30, 2025, the government had set up a six-member expert committee to study the scheme and recommend appropriate revisions. It sought an extension of the original contract with the Oriental insurance company for another three months till it can scrutinise the expert committee's recommendations and float the new tender for the revised scheme The government order (GO) issued by the Finance department extending Medisep for three more months says that the premium charged for the extended period of the scheme will be ₹1,593, including GST, per family for the entire three months period for the existing number of families covered by Medisep. Slightly higher Though this works out to ₹531/month, slightly higher than the current actual premium of ₹472 per month, the government has decided to meet this additional expense from the balance in the corpus fund so that neither the beneficiaries nor the State exchequer will incur any additional payment. Though the insurance company had earlier taken the stand that while extending the contract, it can provide insurance cover to existing families only for whatever balance sum insured remains as on June 30, the State government re-negotiated with the insurance company Thus, families who have already exhausted their sum insured or those whose balance sum insured is less than ₹75,000 as on June 30 will also get a basic insurance coverage of ₹75,000 for the next three months. This has been fixed in proportion to the additional premium paid by the government per family for the extended period. The scheme will provide cashless treatment as well as the benefit of the package to the beneficiaries with the same terms and conditions of the existing Medisep scheme till September 30. Medisep, launched in July 2022, offered a basic insurance cover of ₹3 lakh, at a monthly premium of around ₹500. It covers over 30 lakh individuals, including some 5.45 lakh government employees, 5.89 lakh pensioners and their dependents. Enhanced coverage The expert committee constituted by the government to revise the terms of Medisep had recommended an enhanced basic insurance coverage from ₹3 lakh to ₹5 lakh, based on the framework of the Health Benefits Package (HBP) 2022 formulated by the National Health Authority. It had also recommended that the Medisep policy period be reduced to two years and that the scheme should be extended to employees and pensioners in public sector undertakings, Corporations, autonomous, statutory bodies and the cooperative sector, so that the increase in the overall risk pool can rein in the premium.

Data shared by National Health Authority (NHA) on scheme's dashboard shows that 443 hospitals were empanelled under the AB-PMJAY across India in four months - 161 in Jan, 187 in Feb, 40 in March and 55 in April. In May, the latest update shows, 20 hospitals were empanelled. Many healthcare associations point out that low package rates and delayed payments are key reasons behind the lukewarm response to government schemes from the private sector, especially large corporate hospital chains. However, a senior official of NHA - the implementing agency of the AB-PMJAY - said empanelment is taking place and that some of the new empanelments may not have been updated as they were migrating to a new system. "We may join the scheme if the package rates are increased," he said. The Indian Medical Association has also stressed on the need to increase package rates to make treatment under the scheme more sustainable. "The rates should be raised to at least CGHS level," said an IMA office-bearer. As many as 609 private hospitals have opted out of the scheme since 2018, minister of state for health Prataprao Jadhav said in a written response to a Parliament question in March. "The reasons for hospitals voluntarily opting out include empanelment only for Covid period, hospitals were closed or non-functional, changes in hospital entity, hospital relocated, ongoing reconstruction or renovation, unavailability of specialist doctors, voluntary withdrawal from scheme, package rates, opting out due to certain treatment packages reserved for public hospitals only (Chhattisgarh and Gujarat), and no referral from public hospitals (Karnataka)," he added.
