Putra Heights gas pipeline blast: Excavator not to blame, local council followed SOPs, says Selangor MB
SHAH ALAM, July 8 — The explosion of a gas pipeline in Putra Heights, Subang Jaya on April 1 was not caused by an excavator carrying out digging works that led to the massive fire, the Selangor State Legislative Assembly was told today. Selangor Menteri Besar Datuk Seri Amirudin Shari said investigations by the police, the Department of Minerals and Geoscience (JMG), and the Department of Occupational Safety and Health (DOSH) confirmed that the above-ground work did not impact the gas pipeline, according to a report by national news agency Bernama. He explained that the monsoon drainage pipe located above the gas pipeline remained unaffected. 'Based on my checks with the local authority, the residential area and monsoon drain construction, with a diameter of 4.2 metres, was carried out around 2008 and 2009 — before the establishment of the One-Stop Centre (OSC) in 2010,' he said during a briefing on the Putra Heights incident in the Selangor state assembly. Amirudin also refuted claims that a property developer was carrying out unauthorised construction of shop lots near a Petronas Gas Berhad (PGB) easement. He said the company had in fact received approval to carry out the works on March 20, 2025. 'PGB also required the developer to comply with safety conditions outlined by Petronas during a pre-work meeting before the work permit was issued,' he added. Amirudin dismissed allegations that the Subang Jaya City Council (MBSJ) failed to meet proper criteria when granting planning permissions involving areas under Petronas' supervision or that it compromised buffer zone requirements. He noted that while PGB mandates a 60-foot buffer zone, the designated buffer in the Putra Heights area was 66 feet —exceeding the minimum by six feet. Previously, DOSH's Petroleum Safety Division director, Husdin Che Amat stated that weak soil conditions beneath the pipeline — which failed to adequately support its structure — were the actual cause of the incident. He said investigations found that while the gas pipeline met all technical specifications, the underside of the pipe was unsupported due to soft, damp soil, leading to a vulnerable unsupported segment.

Family tribute to man killed at Brierley Hill industrial premises
The family of a man who was killed at an industrial premises on Tuesday have paid tribute to Storey, 51, died after getting trapped under a piece of steel at a premises on Pedmore Road, Brierley Hill, West Midlands Police Storey's family have paid tribute to Mr Storey, who they described as a "loving partner, son, father, uncle and grandfather"."Tragically taken too soon, Robert will be deeply missed by all who knew him," they said. "His wit, humour and devotion to family will never be forgotten".Mr Storey was in a critical condition when ambulance crews arrived on Tuesday and was given life support, but died at the Midlands Police said they were continuing to work with the Health and Safety Executive to investigate the circumstances around his death. Federal investigators eye driver fatigue in deadly 18 wheeler crash on I-20 tied to Florida-based carrier
Federal investigators are examining the role driver fatigue may have played in a deadly I-20 crash near Terrell over the weekend involving a Florida-based truck driver and his employer, Hope Trans LLC. The National Transportation Safety Board, in coordination with the Texas Department of Public Safety, has opened a safety investigation into the crash that killed five people on Interstate 20 in Terrell, Texas. The driver, identified as Alexis Osmani Gonzalez-Companioni, 27, is from Florida and works for Hope Trans LLC, a trucking company with a listed address in the Orlando area, according to USDOT records. Records show the company had a previously listed address of Tacoma, Washington. Federal transportation records show Hope Trans LLC has been in operation since 2021. The company currently operates 65 trucks and employs 80 drivers. It is listed as being in compliance with federal safety regulations. In the past 24 months, Hope Trans LLC has been involved in three crashes resulting in injuries, though none were fatal until this weekend's incident. In terms of driver compliance, based on federal inspection in the past 24 months, the company's record is slightly better than the national average. Five percent of Hope Trans LLC drivers were found "out of service" during a federal inspection, compared to the national average of 6.7%. The Federal Motor Carrier Safety Administration website shows no "acute/critical" violations for hours-of-service compliance or driver fitness. Federal rules limit commercial truck drivers to a 12-hour driving limit after 8 consecutive hours off-duty, as well as a 70-hour weekly limit over 7 consecutive days. As the NTSB continues its investigation, questions remain about what could have been done to prevent the crash—and whether warning signs were missed.

Factory worker dies 'trapped' in industrial oven in horrifying accident
A Missouri factory worker suffered a horrific death after getting stuck inside an industrial oven. Nicolas Lopez Gomez, 38, a Guatemalan national who worked under the alias Edward Avila, died while cleaning the massive oven at the Gilster Marylee Cereal Plant in Perryville on Thursday around 3pm, police said. He was reportedly power washing the machine from the outside. It is unclear how Gomez ended up lodged inside the turned-off oven, but authorities do not suspect foul play. Panicked co-workers urgently dialed 911 to try to save him once they realized he was trapped. But less than 20 minutes after first responders arrived, the Perry County Coroner was called to the scene and Gomez was pronounced dead, KSDK reported. The coroner's office is working with the Occupational Safety and Health Administration to investigate the harrowing incident. Gilster-Mary Lee Corporation, a private food label manufacturer, has operated for 125 years and has 11 locations across Illinois, Missouri and Arkansas, according to its website. Gomez's unsettling death comes eight months after a teen working at a Canadian Walmart was baked to death inside a walk-in oven. Gursimran Kaur's charred remains were found by her mother inside a bakery oven of a the store she worked at in Halifax, Nova Scotia, on October 19. The mother and her 19-year-old daughter had been living in Canada for about three years and worked at Walmart together. The pair had moved from India, where Kaur's brother and father remain. On the day of the deadly incident, her mother could not find her for over an hour, with her phone unusually turned off and unreachable. Then, a bakery section employee pointed out a 'leakage' from inside the walk-in oven - which led Kaur's mother to make the gut-wrenching discovery of her burned-to-death daughter. By the time emergency responders arrived at the scene, Kaur had died inside the industrial oven. Police ruled out foul play in November, but the store was closed until February 3 for renovations. 'It's great to have our customers and our associates back in the store,' Nick Ritcey with Walmart Canada's corporate affairs told CBC at the time. 'Obviously the tragedy is still top of mind and it's something that will be sad forever.' After police said the Halifax store could reopen, Walmart confirmed they would remove the walk-in oven. Walmart spokesperson Amanda Moss told the National Post: 'This is an extremely sad and difficult situation. 'Removing the oven had always been part of the standard remodel program we are implementing across the country. 'Now that the stop-work order has been lifted by the Department of Labor, the oven will be removed from this store and will no longer be used.'

Explorers offer peek at abandoned theme park where boy, 13, died in tragic accident a decade ago
Urban explorers have shed light on an eerie abandoned theme park where a teen died tragically more than 10 years ago. Video footage shows the decaying Copa Copana Water Park in Athens - seemingly unchanged from the 2014 disaster that claimed the life of the 13-year-old boy. He and his sister, nine, were swept up by a gust of wind while playing inside a large inflatable ball at the 32,000 sq ft site in the Greek capital. Decaying: The park was the site of one of the grimmest amusement park disasters in 2014 when a boy died while enjoying one of its attractions They were flung from the surface of a pool about 100ft into a metal barrier and rushed immediately to Agia Sofia Children's Hospital in Athens. The boy died shortly after, having suffered severe head injuries, while his sibling was left critically injured and diagnosed with severe lung contusion. Copa Copana's two managers were arrested and an investigation began into whether safety issues played a role in the tragedy. Greek police concluded it was a result of both 'bad weather conditions and human error' and the park was swiftly closed for good. Now a group of urban explorers have headed back to the park and discovered a creepy site, with the attraction's rides and slides still up, but barely a soul around. Nature has reclaimed the abandoned park, with huge spiraling plants intertwined with the twists and turns of the various defunct flumes. With the facility tucked in between a hillside and a main road, the YouTubers - whose account Urbandoned has 138,000 subscribers - try to sneak into the site from the rear. After a walk through a forest of thick trees, the duo eventually see an abandoned ride peeking out from the undergrowth. The park is well protected, with a layer of razor wire coating a fence and a small surrounding road, seemingly used by security staff. They eventually squeeze beneath a small gap in the fence before exploring the spooky site. Discarded piping and metal cover the park's grounds with thick weeds and plants dispersed between what were once popular rides. Drone footage shows the slides gathering rust and moss and pools of stagnant water, filled with tadpoles, sitting in sections of the facility. A climb to the top of one of the flumes offers expansive views of the surrounding mountains, city and the meandering metal rides. As he walks down a path, overgrown with towering green plants, one of them describes Copa Copana as 'an actual jungle.' The narrator explains that the park in its heyday boasted 13 rides as well as cafes and restaurants. Bizarrely, they spot dozens of sun loungers and outdoor tables stowed away beneath one of the slides, a reminder of the fun experiences once had at the site. It turns out they are not the first explorers to visit the abandoned park, with several rides seen covered in graffiti, sprayed on in the years since they were drained out. A trip up a few flights of creaky stairs to the highest point in the facility allows one of the YouTubers to even get a spectacular view of the ocean, as well as the vast extent of the creepy site. Poignant archive footage then shows Copa Copana at its happy peak several decades before, with hundreds of children and parents enjoying its attractions. The group are initially surprised by the lack of security at the abandoned site but it turns out the guard had been resting on a sun lounger before ambling towards them. The friendly member of staff allows them to stroll to the part of the park they have not yet seen as long as they then head for the exit. Eerily, some of the lifeguard chairs and even cafe menus remain left exactly where they stood during Copa Copana's operational days.
